It’s easy to enroll with Easy Wireless. Click on your state of residence to begin the online application process. You will be guided through a series of questions necessary in determining your eligibility for Lifeline. Complete the form, attach all necessary documents of proof and identification, electronically sign the form and our customer service reps will quickly review your application and contact you to arrange receipt of your free phone if you qualify.

If you’d rather download our application forms, you can either fill them out on your computer or print the application first and fill it out by hand. Once you have finished, please sign and mail it back to us or fax it to us. You may also scan the document and attach it to an email to send it to us. Please note that a completed application with supporting documentation is required in order for us to process your request and see if you qualify for a free phone under the Lifeline program.
